OVERVIEW

We are seeking a compassionate, organized, and motivated Supported Community Living (SCL) Coordinator to oversee individualized support plans and ensure quality services for individuals with disabilities. This role involves coordinating care, monitoring staff performance, maintaining accurate documentation, and collaborating with clients, families, and support teams to promote independence, social skills, and personal growth.

RESPONSIBILITIES

• Develop, coordinate, and monitor Individual Program Plans (IPPs) and behavior support programs.
• Evaluate services and ensure compliance with organizational, state, and accreditation standards.
• Maintain accurate client records and review documentation for compliance.
• Collaborate with clients, families, staff, and service providers to meet individual needs.
• Facilitate interdisciplinary team meetings and monitor progress toward client goals.
• Supervise and support SCL staff, including onboarding and documentation training.
• Assist individuals with daily living activities and transportation as needed.

QUALIFICATIONS

• Bachelor’s degree preferred, or an equivalent combination of education and experience.
• At least one year of experience working with individuals with disabilities.
• Strong communication, organizational, and interpersonal skills.
• Ability to develop individualized, person-centered support plans and maintain confidentiality.
• Valid driver’s license, current auto insurance, and reliable transportation.
• Ability to pass required background checks.
• Willingness to obtain Medication Management, CPR, and Safety-Care certifications within the first six months of employment.
